The Napa Post Acute Mission
The mission at Napa Post Acute is twofold—to be a skilled nursing facility with skillful clinical capabilities and create a home away from home for the residents we care for. We strive to make the adjustment to a post acute center easy so every resident feels welcome, supported, and cared for. At Napa Post Acute, we believe these are not simply patients. They are someone’s mother, father, daughter, son, or loved one. Our management team approaches care with the same compassion and attention we would want for our own family.

Our interdisciplinary team works together across nursing, rehabilitation, social services, activities, dietary, admissions, and other departments. Collaboration helps us understand each resident’s needs and create an individualized approach to care.

Therapy / Rehabilitation
Our rehabilitation professionals provide physical, occupational, speech, respiratory, and other therapy services designed around each resident’s needs and goals.
